I have discussed the options for the payment of my holiday pay and I request that Sandbeck Umbrella make advances against my accrued annual paid leave reserve fund each time I am paid. I understand that holiday pay will be paid to me weekly as an addition to my weekly pay. I understand that in order to take annual leave I should submit a request for my holiday entitlement using the normal holiday procedures but, I acknowledge that I have received my holiday pay in advance and will not receive any additional payment for time spent during any period of annual leave.I would like my Holiday Pay to accrue as normal and to be paid to me whilst I am on annual leave.
